Follow Up

Your dietitian will track your progress, update your plan, troubleshoot challenges, and get continued support with new strategies and tools.

Intermediate Package

- Initial assessment session
- 3-week individualized meal plans with recipes and grocery list
- 5 follow-up sessions to review meal plans and progress
Health Support Package

- Initial 60-minute consultation with tailored education on diabetes and/or heart health
- 3 customized meal plans
- 5 follow-up sessions covering: Nutrition label reading, Mindful eating, Carbohydrates, glycemic index, and fat types and Lifestyle strategies
